Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following prescribed protocols while ensuring compliance with HIPAA regulations and safety standards.
- Assist with complex medical procedures such as IV infusion, catheterization, wound suturing, and sterile processing.
- Monitor patients’ conditions closely, including those in critical care units like ICU or Level I Trauma Centers, and escalate concerns promptly to physicians or senior nursing staff.
- Support discharge planning, case management, and coordination of care for patients with chronic conditions such as dementia or Alzheimer's disease.
- Provide specialized care for diverse populations including pediatric, neonatal ventilator management, geriatric patients in memory or hospice care, and individuals with developmental disabilities.
- Maintain accurate medical documentation of patient care activities, medication administration, and diagnostic evaluations using EMR/EHR systems like Athenahealth or eClinicalWorks.
Requirements
- Valid LPN license with current state licensure in good standing.
- Proven experience in various healthcare settings such as hospitals (including Level II trauma centers), nursing homes, outpatient clinics, or home care environments.
- Familiarity with advanced nursing procedures including IV insertion, tube feeding, ventilator management, and pain management techniques.
- Knowledge of ICD-9/10 coding systems and medical terminology relevant to acute care and specialty areas like cardiology or infectious disease care.
- Strong understanding of infection control practices, HIPAA compliance, and patient safety protocols.
- Experience working with EMR/EHR systems such as Epic, Cerner, or Athenahealth for accurate medical records management.
