Responsibilities
Our Reablement Assistants promotes independence by meeting the individual’s needs. It’s important to us that we support our customers in the community with a collaborative and person-centred approach.
Our customers will need support with daily living task such as personal care, medication administration, meal preparation, transfers. Encourage physical, mental & social activities, monitor & report on progress and wellbeing.

We are looking for a caring and reliable D2A Enablers to support individuals in their recovery process. You'll play a key role in helping people rebuild confidence and independence, working alongside with occupational therapists, district nurses, physio therapist, key workers and other healthcare professionals. Must held a valid driving licence and access to own vehicle.
