Position Summary
Blue Zones Health Centers deliver primary care and lifestyle medicine to improve patient quality of life and patient health outcomes, while improving their satisfaction. Centers also serve as a test kitchen in developing a scalable model to provide lifestyle-driven primary care. This Advanced Practice Provider will provide primary care for a panel of patients, and work with the well-being team to improve lifestyle medicine delivery.
The compensation range for this role is $110,000 - $120,000 annually.

Key Responsibilities
- Provide direct patient care for a panel of patients, either in person or through telehealth (audio and/or visual)
- Utilize Lifestyle Medicine principles (nourishment, movement, renewal, connection) to prevent, treat, and reverse chronic illness where needed
- Coordinate patient care with other agencies and interdisciplinary care teams as needed
- Manage their own inbox
- Provide on-call clinic coverage
- Cover for other providers’ patient care and inbox management as needed
- Participate in quality improvement processes as needed
- Maintain accurate documentation and comply with all regulatory standards
- Other duties as needed and mutually agreed upon
Qualifications
- Licensed Nurse Practitioner (FNP/AGNP/ANP) or Physician Assistant (PAC) with active, unrestricted U.S. license; national certification in good standing. ACLM (American College of Lifestyle Medicine) Certification preferred
- 2 years adult clinical care experience.
- Proven clinical assessment and decision-making skills
- Proven experience with telehealth platforms and EHR systems
- Passionate about helping patients make sustainable lifestyle changes
- Team player, flexible and adaptable with strong time management skills
- Conversational Spanish preferred
- Ability to travel as needed
