Position Summary/Responsibilities
Case Managers in our Assisted Living Facilities (ALF) program provide care coordination services to individuals who reside in or recently resided in an Assisted Living Facility. Services are person-centered and designed to assist an individual with a severe mental illness in accessing basic needs.
A Day in the Life of an ALF Case Manager:
ALF Case Managers have a lot of contact with clients, as they usually visit each client at least monthly. They help their assigned clients get connected with services they need and qualify for, so they frequently communicate with Social Security, Social Services, and Disability providers to assist the individuals with their benefits. They work closely with staff from the assisted living facilities, including nurses and facility administrators, regarding doctor appointments, medications, and any issues that arise.
Hiring Range
The anticipated base salary for this position is $43,022.07 to $45,890.21. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.
Requirements
- Education: A minimum of a high school diploma. An Associate's or Bachelor's degree in a human services field preferred.
- Experience: Experience working with adults with serious mental illness is a plus.
- Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
- Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
Special Work Conditions
While performing the duties of this job, the employee will often sit, stand, walk, bend, stoop, crawl, climb and reach above their head. Use of hands for placing and pulling objects. The employee frequently lifts and or carries up to 25 pounds and pushes or pulls up to 150 pounds. Specific vision abilities required by this job include close vision (computer). Specific hearing required by this job includes normal tones. Travel up to 6%. Travel is primarily local.
