About the Role Our Physical Therapists in the Home Care setting evaluate care plans and deliver comprehensive therapy to patients in their homes, helping restore function and independence through rehabilitative modalities as prescribed by a physician.
Key Responsibilities:
· Perform thorough, accurate patient assessments
· Complete OASIS data sets for “therapy only” cases
· Communicate clinical changes to the care team in a timely manner
· Coordinate care planning with providers, patients, families, and the care team
· Assess and recommend appropriate durable medical equipment and adaptive equipment
· Create individualized home exercise programs based on patient needs
· Supervise and mentor Physical Therapy Assistants
· Maintain accurate documentation and submit timely visit verification
· Stay informed of community resources and refer as needed
· Independently manage appointments and caseload throughout Grant County
Qualifications Required:
· Valid and current Oregon Physical Therapy license
· Graduate of an accredited PT program (as recognized by APTA or equivalent accrediting body)
Preferred:
· Doctorate in Physical Therapy
