As the Redirection of Care Specialist with BCBSWY, you serve as an essential member of the Population Health team supporting members in their healthcare journeys. This role requires the ability to support a member-centric approach through collaboration with internal and external partners. This role also requires the ability to build rapport and foster positive productive internal and external relationships.
As a member of the Population Health Team and an employee of BCBSWY, you are an integral member of our operational organization. You will be responsible for managing redirection of care initiatives including system programming, documentation, company-wide communication, and internal plus external relationships. You will step into oversight of the Redirection of Care program by processing authorizations and education and problem solving with members and providers to support positive outcomes.
To be successful in this role, you will need knowledge of clinical program effectiveness and the ability to assess outcomes. Competency with data analysis, interpretation, and application will play a crucial role in the day-to-day activities assigned.
Minimum qualifications include an associate's degree in nursing and an active Wyoming RN license and 1 year of general nursing experience.
There may be evening and weekend hours on a periodic basis and moderate in-state travel, with some overnight travel possible. As a recognized BCBSWY employee, we count on you to positively serve in the role of "Brand Ambassador" within the local community and/or geographical region.
