Bloomer Health is a tech-enabled Early Intervention agency servicing NYC, Westechester and Long Island. We provide service coordination and service delivery to babies and toddlers aged zero to three years old. Our agency uses an innovative platform built by providers, for providers, which allows for easy onboarding, case management, documentation and billing. Our mission is to help modernize and reduce time spent on non-clinical tasks in an effort to allow more time for what's important: supporting families. We actively assist the service coordinator from behind the scenes to ensure ease of documentation, activity logging, billing and case management. With an in-house tech team, we are constantly improving our system to allow our coordinators to be more efficient, organized and streamlined!

Job Overview
Bloomer Health is seeking dedicated and detail-oriented Service Coordinators to support families of infants and toddlers (ages 0–3) who are referred for Early Intervention services. In this role, you will play an integral part in guiding families through the evaluation and service delivery process, facilitating connections with providers, and ensuring each child receives the coordinated support necessary to achieve optimal developmental outcomes.
This position is well-suited for qualified individuals who are organized, detail oriented, enjoy working with people; and tech-savvy.
Responsibilities
- Serve as the primary point of contact for families enrolled in Early Intervention services
- Conduct intake and ongoing assessments of family needs and priorities
- Develop and maintain Individualized Family Service Plans (IFSPs)
- Coordinate services across multiple providers and disciplines
- Monitor service delivery to ensure compliance with state and program guidelines
- Maintain accurate and timely documentation using the Bloomer Health digital platform
- Support families by connecting them to community resources and services
- Facilitate communication between families, therapists, and other stakeholders
- Background in social work, behavioral health, childcare, or related fields preferred
- Excellent communication skills to engage with diverse stakeholders including families, legal professionals, and community agencies
- Ability to handle sensitive situations with empathy and professionalism while maintaining confidentiality
Qualifications
- Bachelor’s degree required (Master’s degree preferred) in Social Work, Psychology, Early Childhood Education, or a related field
- Prior experience in Early Intervention, case management, or service coordination (preferred)
- Strong organizational, communication, and interpersonal skills
- Ability to manage multiple cases and deadlines efficiently
- Experience working with children ages 0–3 and their families (preferred)
- Knowledge of IFSP development and Early Intervention systems (a plus)
- Comfortable using digital tools for documentation and communication
