Position: Physical Therapist/Physical Therapist Assistant.
Location: Contra Costa and Alameda Counties.
Job Summary:
We’re looking for an experienced and compassionate Physical Therapist or Physical Therapist Assistant to join our team. You will provide therapy services to patients in their homes, helping them restore function, improve mobility, and achieve their personal goals.
Responsibilities:
- Assess patients’ conditions and review medical histories and referrals.
- Develop and implement individualized treatment plans.
- Teach patients exercises and stretches to improve strength and mobility.
- Maintain accurate records and track progress.
- Collaborate with patients, families, and healthcare professionals.
Qualifications:
- Minimum 1 year of physical therapy experience (home health preferred).
- Strong communication and teaching skills.
- Compassionate and professional demeanor.
- Able to lift up to 50 lbs as needed.
Requirements:
- Valid PT or PTA license.
- Basic Life Support (BLS) certification.
- Valid driver’s license.
