Blize Healthcare is seeking a compassionate and driven Hospice Community Liaison to join our growing team. This role focuses on building and nurturing relationships with referral sources - including physicians, hospitals, skilled nursing facilities, and community organizations.Location: Contra Costa and Alameda Counties.Key Responsibilities:Develop and maintain strong relationships with healthcare providers and community partners.Identify and secure new referral sources through education and outreach about hospice services.Represent Blize Healthcare at trade shows, community events, and professional gatherings.Educate the community about the benefits and philosophy of hospice care.Monitor, track, and analyze referral and admissions data to identify trends and opportunities for growth.Collaborate closely with the clinical team to ensure a seamless transition to hospice care for new patients.Skills and Qualifications:Strong communication, interpersonal, and presentation skills.Proven ability to develop and maintain professional relationships.Sales or marketing experience, preferably in the healthcare industry.Knowledge of the hospice care continuum, services, and regulatory requirements.Associate’s degree or equivalent experience.Physical capability to lift up to 50 pounds, as needed.Reliable transportation for travel within assigned territories.Benefits:Competitive bonus structure.Health, dental, and vision insurance.Paid time off.Mileage or fuel reimbursement.
