To provide care management services to eligible clients, in which an individual's needs are identified and the psychosocial and medical services designed to meet those needs are located, coordinated and monitored in collaboration with other professional staff. Using a biopsychosocial assessment to identify actual needs and potential risks for adverse outcomes, the Social Worker coordinates access to community-based services to meet the needs and risks identified. Develops, implements, coordinates and evaluates client service plans in an outpatient environment; collaborates with other health care providers, community service agencies, faith-based agencies, and schools, as appropriate, to coordinate care and services. This position requires full understanding and active participation in fulfilling the Mission of the Blessing Health System. ICARE standards must be demonstrated. Adherence to Code of Conduct and other policies is required.

Education/Training/Experience
JOB QUALIFICATIONS
Required
- BSW with Social Worker License or MSW.
- LCSW.
- Broad range of experience, including discharge planning, disease management and previous work in skilled/acute care facilities.
- Current Illinois Social Worker License within 6 Months.
- NON-EXEMPT
