What are the responsibilities and job description for the Community Outreach Worker position at Blessing Care Services, LLC?
Overview:
In this role, you will be responsible for building and maintaining long-term relationships between Blessing Care Services, LLC, and community referral resources; ensuring consumers receive services in accordance with ethical and legal requirements; and ensuring consumer retention through outreach via phone calls, texts, emails, and surveys.
Duties:
· Build and maintain relationships with community referral sources, such as organizations, medical facilities, social services, schools, and any other service providers
· Complete new client's admission paperwork, such as screening and/or intake by telephone or in person
· Receive and process referrals and schedule appointments.
· Comply with HIPAA & state and federal policies and procedures related to home health care and behavioral health services
· Identify and target relevant referral sources using the DHHS list of community potential referral resources
· Contact medical facilities, care coordinators, and care managers for potential referrals
· Utilize various resources such as brochures, social media, community outreach opportunities, and partnerships, to reach potential clients.
· Conduct a search to stay updated on industry trends and competitor activities.
· Make cold calls to varied potential referral sources such as group homes, crisis centers, hospitals, and others to establish contact and concrete eligible referral sources and secure consumer referrals.
· Make in-person visits to community resources to foster relationships and secure consumer referrals.
· Provide an average of two consumer referrals every week.
Experience and qualifications:
· At least one year of work experience in behavioral health and/or home care services
· Ability to work collaboratively with professionals for the best care of consumers.
· Strong communication, interpersonal, and problem-solving skills.
· Ability to work independently, prioritize tasks, and meet deadlines.
· Knowledge of healthcare regulations and compliance standards is desirable.
· Associate's, bachelor’s, or master's degree in human services or related field
Pay: $100.00 - $500.00 per week
