- Develop, coordinate, and oversee individualized care plans for homecare clients
- Conduct client assessments and regular check-ins to monitor health, safety, and well-being
- Serve as the primary point of contact for clients, families, and caregivers
- Supervise and support direct care staff to ensure high-quality service delivery
- Coordinate with healthcare providers, social workers, and community resources
- Maintain accurate client records and documentation in compliance with regulations
- Respond to client concerns, incidents, and changes in care needs in a timely manner
- Experience in homecare, case management, or a related healthcare field
- Strong knowledge of care planning, patient advocacy, and community resources
- Excellent communication and interpersonal skills
- Ability to manage multiple cases and prioritize effectively
- Compassionate, patient-centered approach to care coordination
- Proficiency with documentation and basic computer applications
- Valid driver's license and reliable transportation preferred
