What are the responsibilities and job description for the Health Care Navigator position at Blackstone Valley Comm?
The Care Team Navigator is responsible for providing outstating patient care in the primary care/medical home model environment. The goal of this role is to assist the care team in closing care gaps, tracking and follow up of referrals and orders effectively and efficiently processing prior authorizations, and provide backline support for the call center. The Care Team Navigator displays responsible behaviors, communicates effectively with others including outside entities and functions as a member of the healthcare team. The care team navigator functions under the direct supervision and authority of the Physician/Licensed Independent Practitioner when performing clinical tasks during patient care.
EDUCATION, EXPERIENCE, & SKILLS
Experience as a Medical Assistant preferred.
Experience with medical terminology
A courteous and cooperative attitude
OTHER REQUIREMENTS
Reliable transportation required.
Bilingual ability in English and Spanish, Portuguese or Creole speaking abilities preferred.
Cultural sensitivity is necessary to collaborate with a diverse patient and staff population required.
Ability to work independently and collaboratively is required.
Knowledge of computers and electronic medical records required.
Knowledge of Microsoft Suite preferred.
Effective communication skills, both verbal and written required
