Position Summary
Blackstone Home Healthcare is seeking a detail-oriented and organized Medical Intake Coordinator to manage the intake and referral process for new patients. The Intake Coordinator serves as the primary point of contact for referral sources, including hospitals, physicians, case managers, and insurance providers. This position is responsible for verifying patient eligibility, coordinating admissions, and ensuring timely communication between the clinical team and referral partners.
The ideal candidate has experience in home health, medical intake, insurance verification, or healthcare administration and is comfortable working within an electronic medical record system such as WellSky.
Key Responsibilities
Referral Intake & Processing
- Receive and review patient referrals from hospitals, physicians, case managers, and other referral sources.
- Enter referral information accurately into the agency’s electronic medical record system.
- Verify patient demographics, insurance coverage, and eligibility.
Insurance Verification & Authorizations
- Verify insurance benefits for Medicare, Medicaid, private insurance, and managed care plans.
- Obtain and document required authorizations prior to the start of care.
- Communicate authorization requirements to clinical staff.
Admission Coordination
- Coordinate with the Director of Nursing (DON) and clinical staff to schedule Start of Care visits.
- Ensure required documentation is received before admission.
- Communicate admission status updates to referral sources.
Communication
- Serve as a liaison between referral sources, patients, families, and clinical staff.
- Maintain professional communication with hospitals, discharge planners, and physician offices.
- Provide updates regarding referral status and patient admissions.
Documentation & Compliance
- Ensure all intake documentation complies with Medicare, Medicaid, and agency policies.
- Maintain accurate and organized referral records.
- Support compliance with ACHC accreditation and regulatory requirements.
Administrative Support
- Assist with scheduling and coordination of patient visits when needed.
- Maintain intake logs and referral tracking reports.
- Participate in team meetings and process improvement initiatives.
Qualifications
Required
- High school diploma or equivalent
- Minimum 1 year healthcare administrative or intake experience
- Strong computer skills and experience with medical software systems
- Knowledge of insurance verification processes
- Excellent communication and organizational skills
Preferred
- Experience in home health or hospice intake
- Experience with WellSky (Kinnser) EMR
- Knowledge of Medicare and Medicaid home health eligibility
- Medical billing or coding familiarity
