Hours: Primarily Monday through Friday daytime hours with opportunities to have a flexible schedule

General Description of the Position:

This position provides comprehensive case management services to individuals with physical, intellectual, and developmental disabilities. It focuses on promoting independence, coordinating care, and advocating for access to community-based services that support each participant’s health, safety, and overall quality of life. Nurse case managers support a caseload of adult participants with diverse intellectual and developmental needs who reside in group homes, apartments, and private residences throughout the Rapid City community. They facilitate medical services by working closely with Direct Support Professionals—non-medical staff who provide daily care in the participants’ homes. As a key member of each participant’s care team, the nurse case manager plays a vital role by offering medical guidance, training, and ongoing support to ensure safe and effective care.

Qualifications:

• Current nursing licensure in South Dakota.  
• Current BLS Certification 
• Valid driver's license.
• Clean background check and drug screen

Primary Responsibilities:

1. Provides information and assists team in developing programs to enhance an individual's community integration.
2. Provides consistent communication with medical providers to advocate and follow up with medical concerns. 
3. Provides training and consultation to non-medical personnel.
4. Completes infection Control Program for the agency, maintaining logs and making recommendations as required to assure high standards of safety.
5. Confirms upcoming appointments and refers to MSA (Medical Support Assistant) to schedule.
6. Attends specialty medical appointments as needed.
7. Completes appointment medical data forms and inputs into computer record and carries out doctor's orders. 
8. Maintains current record of all health-related appointments on each participant in his/her caseload in a nursing medical file.
9. Accepts and documents physician orders. 
10. Documents medication, dietary, treatment and all health-related orders and assures that the participant, staff, and guardians or other caregivers are aware of new or changed interventions.
11. Performs medical assessments and addresses emergency needs as they arise. 
12. Performs IM injections, Foley catheter insertion, supra pubic catheter changes and non delegatable wound care for participants as required per certified medical professional orders.
13. Maintains strict confidentiality of health information.
14. Provides staff training to assure health needs of participants are met such as medication administration certification classes and others.
