Come join our team as a full-time Care Coach with BJC Medical Group in Internal Medicine, this is a rare opportunity to join a great team!
- Outpatient
- 40-Hours per week
- Day Shift M-F 8 a.m. - 4:30 p.m.
- 3-4 weeks in person training
- Remote with - on-site 3 to 4 times per year
- Medical Assistant and LPNs preferred

Role Purpose
The Care Coach, in collaboration with the interdisciplinary care team, is responsible for helping facilitate patient care along the continuum through effective transition planning. This position assists patients with achieving optimal health, access to care and appropriate utilization of resources to ensure patients receive the right care at the right time.
