Registered Nurse (RN) - Home Health

Bios Home Health, an employee-owned company, is hiring a full-time Registered Nurse (RN) with skilled home health experience to provide patient visits in Sapulpa and surrounding areas. This position offers per-visit pay, a Monday–Friday schedule, a company-provided gas card, and employee benefits.

Visit Pay

• Start of Care (SOC): $110 per visit
• OASIS Recertification, Resumption of Care, and Discharge: $77 per visit
• Skilled Nursing Visit (SNV): $45 per visit

Schedule and Travel

• Full-time position
• Monday–Friday schedule
• Patient visits in Tulsa and surrounding areas
• Company-provided gas card for work-related travel

Documentation and Clinical Support

We use Homecare Homebase (HCHB) for clinical documentation. Apricot AI assists with OASIS Start of Care documentation.

Onboarding includes training on HCHB, OASIS assessments, Paycom, and tablet use. Training needs and duration depend on your prior home health and software experience.

You’ll work independently in patients’ homes while coordinating care with our nursing leadership, physicians, therapists, and caregivers.

Your Responsibilities

• Complete comprehensive patient assessments.
• Perform Start of Care, OASIS recertification, resumption of care, and discharge assessments.
• Provide skilled nursing visits, including medications and treatments as ordered.
• Assess wounds, provide wound care, and monitor healing.
• Educate patients and caregivers on disease management and home care.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Maintain timely, accurate clinical documentation.

Qualifications

• Active Oklahoma Registered Nurse license
• Skilled home health experience
• Strong assessment, communication, and documentation skills
• Valid driver’s license, reliable transportation, and current auto insurance
• Ability to travel between patient homes
