What are the responsibilities and job description for the Family Model Provider position at BIOS CORPORATION?
Bios is seeking compassionate individuals or families in Tennessee to serve as Family Model Caregivers and provide shared living support for an adult with intellectual or developmental disabilities. As a Family Model Caregiver, you welcome someone into your home and help them live a meaningful, community-based life while offering daily support, structure, and supervision.
Responsibilities include:
- Providing daily support, structure, and supervision in your home (including 24-hour support as needed)
- Ensuring the individual’s health, safety, and emotional well-being in accordance with their Person-Centered Support Plan (PCSP)
- Assisting with daily living activities such as meals, personal care, medication administration (if certified), and transportation to community and recreational activities
- Encouraging independence, choice, and participation in meaningful community activities
- Supporting personal skill-building, inclusion, and community engagement
- Maintaining clear, accurate documentation, progress notes, and required reports in accordance with MCO standards
- Communicating and collaborating with case managers, support teams, and family members to ensure consistent, person-centered outcomes
- Participating in required training, monitoring visits, and quality reviews
Qualifications:
- Must reside in the state of Tennessee
- Must be at least 21 years old
- Must pass background and registry checks
- Must maintain a safe, clean, and welcoming home environment
- Willing to complete required DDA/MCO training (provided)
- Valid driver’s license, reliable transportation, and car insurance
- Compassionate, patient, and comfortable supporting adults with varying needs
- Home/Renters insurance is required
