Job Description
- Be a member of multidisciplinary team,
- Perform case management duties.
- Writing weekly/monthly notes, communicating with patients' families, participating in treatment planning.
- Minimum LSW.
- Case management.
- Bachelor's or master's Degree in social work.
- 8:00 AM 4:00 PM
- Lunch (unpaid): 12:00 -1:00
- Dress Code: Business casual
