The incumbent in this position is responsible for assisting in the development, implementation and delivery of high quality pharmaceutical services. Work of the graduate pharmacist is conducted under the review and supervision of the registered pharmacist.
Essential Job Functions
Participates in day-to-day clinical patient care as appropriate for practice area
Reviews and interprets provider orders to ensure appropriate medication use. Contacts provider to clarify any questionable order.
Carefully checks all data entry, medication carts, and ADM fills for accuracy as appropriate.
Accurately and safely, prepares or supervises the preparation of all sterile (e.g., IV admixtures) and non-sterile products.
Controls the distribution of controlled substances, dangerous drugs, and investigational drugs as required by state and federal regulations.
Assists in the training and orientation of new employees, students, and residents. Provides assistance, direction, and clinical supervision of the pharmacy technical staff as appropriate for practice area.
Document in the medical record (and other applications) and is accountable for all clinical pharmacy work including:
Monitoring drugs, disease states, and clinical pathways and interacts with providers to optimize drug therapy; writes consults in progress notes
Recommends therapeutic alternatives to providers to increase therapeutic efficiency and reduce cost
Document consults in designated patient chart or medical record
Acts as a drug information resource to medical staff, other professionals, patients, and their families.
