A Hospice Clinical Liaison serves as a critical link between potential patients, their families, referral sources, and the hospice care team, educating about hospice services, assessing eligibility, and facilitating the admission process. Key responsibilities include establishing strong relationships with hospitals, doctors, and community partners to promote hospice services, communicating patient needs to the interdisciplinary team, and ensuring a smooth transition into hospice care while providing ongoing support and education to patients and families. Key Responsibilities
- Patient & Family Education: Educates patients and their families on the benefits and scope of hospice care, providing comfort and support during their life changes.
- Referral Management: Responds to patient referrals, reviews medical records, and conducts pre-screening to determine hospice eligibility.
- Liaison & Relationship Building: Serves as the primary point of contact between the hospice agency and referral sources, building strong relationships with physicians, hospitals, skilled nursing facilities, and other healthcare providers.
- Care Coordination: Works with patients, families, and the interdisciplinary team to develop and coordinate a care plan that aligns with the patient's wishes.
- Marketing & Sales: Promotes hospice services and educates the community about the benefits of hospice care to generate new patient referrals and meet enrollment goals.
- Internal Communication: Communicates patient needs and care plans effectively with the interdisciplinary team to ensure smooth operations and excellent patient care.
- Community Engagement: Represents the organization at community events and professional meetings to increase visibility and build relationships.
