Duties

• Perform initial and ongoing nursing or personal care assessments for hospice and palliative patients.
• Evaluate pain and symptom management changes/outcomes in coordination with the Hospice Medical Director or on-call physicians.
• Implement and revise individual plans of care based on the patient's changing status.
• Deliver scheduled and as-needed home or facility-based visits over the weekend.
• Provide hands-on clinical care, palliative comfort measures, teaching, and counseling to patients and families.

Skills

• Critical care experience with a strong background in ICU, emergency medicine, or trauma centers enhances your ability to handle complex cases confidently.
• Strong patient assessment skills combined with physiology knowledge enable precise clinical evaluations.

Pay: $75.00 - $150.00 per day
