Clinical Hospital Liaison—Home Health

The Hospital Liaison provides comprehensive support and direction to facility staff in delivering nursing services to patients and their families, coordinates case management of patient services and medical care, and works within an interdisciplinary care setting to assist and support patients and families through the dying process.

Duties/Responsibilities:

• Respond to direct requests for assistance, referrals, and education within assigned hospitals and other facilities or doctors officies.*
• Utilize informal and impromptu opportunities to educate, support, and develop solution driven resolutions that support positive partnerships with hospital resources and other providers and contacts.*
• Provide support and direction to facility staff in delivering nursing services to patients and families that may benefit from hospice and home health services.*
• Coordinate case management of patient services and medical care for those who may qualify for hospice and home health services.*
• Work in an interdisciplinary care setting to teach, assist, and support patients and families through the rehabilitation and end-of-life care continuum.*
• Participate with and provide feedback to the Team Manager and Director of Home Healthcare and other department personnel in the development and implementation of plans for ongoing hospice and home health focused education and outreach.*
• Works closely with BBH staff and providers to coordinate admission of GIP patients to our inpatient units.
• Partners with hospital on discharge planning process including transportation, medications, DME, etc.*
•  Conducts daily visits to case managers within the hospitals (HCA and TMH) to inquire about potential referrals or business opportunities.*
• Participates in cross training with the community educators in the field.*
• Hosts quarterly lunch and learns with case managers at the hospitals.*
• Utilizes the Customer Relationship Management (CRM)tool for documentation of efforts, contacts and outcomes. *
• Coordinates and obtains updated physician orders from hospital providers and provider offices to facilitate timely starts of care and resumptions of care.
• Regular meetings with Intake Coordinator and Director of Home Healthcare

Education and Experience:

• Bachelor's degree in healthcare or related field.
• Current State of Florida or eligible Compact RN, LPN, MSW or COTA license required.
• Experience in hospice, palliative or home health care preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in an interdisciplinary team setting.
• Compassionate and supportive approach to patient and family care.
• Ability to work assigned weekends and on-call shifts.
