Family Promise of the Big Bend is seeking a committed Permanent Supportive Housing (PSH) Case Manager to help individuals with disabilities and low incomes secure long-term housing stability. This position plays a direct role in strengthening lives and strengthening our community.

This is a temporary position funded through a one-year grant. Employment is expected to continue for the whole grant period, with the possibility of extension if additional funding becomes available. The role is ideal for someone who wants to make an immediate, measurable impact.

Job Summary

The PSH Case Manager provides direct support, case management, and monthly home visits for clients living in scattered-site housing. The role focuses on promoting housing stability, crisis intervention, and access to essential services.

This is a hybrid position: Remote work is permitted for documentation and administrative tasks; in-person work is required for home visits, team meetings, and client support.

Responsibilities

  • Provide consistent case management and monthly home visits for PSH participants.
  • Support clients in maintaining housing and developing skills that encourage independence.
  • Offer crisis intervention and ensure timely connection to community resources.
  • Maintain strong relationships with landlords, service providers, and partners.
  • Complete accurate documentation in alignment with program and funder requirements.
  • Work collaboratively with the housing team to deliver high-quality, client-centered services.

Physical Requirements

  • Ability to remain on your feet for extended periods.
  • Ability to walk long distances, including during home visits and outreach.
  • Ability to lift and carry up to 25 lbs (files, equipment, supplies).
  • Reliable local travel and a valid driver’s license are required.

Required Qualifications

  • Bachelor’s degree in Social Work, Psychology, or a related Human Services field.
  • Minimum 2 years of human services experience (housing, shelter, case management, direct care).
  • CPR/First Aid certification, or ability to obtain within 30 days.
  • Strong organizational, communication, and crisis-response skills.
  • Proficient in Microsoft Office and comfortable working in databases.
