Hours: 40 hours/week
Reports To: HRSN Program Manager

Position Summary
Under the supervision of the Health-Related Social Needs (HRSN) Program Manager, the HRSN Case manager will provide bilingual and culturally specific services to Latinx and other communities.
The HRSN Case Manager will engage with community members who participate with OHA CCOs and Acentra medical insurance via referral systems. The HRSN Case manager will provide housing support to individuals with complex medical needs by building rapport, providing tenancy support, short-term case management, Outreach & Engagement, and other services alike through a trauma informed and housing first model lens. The person in this position will use UniteUs and other closed loop referral systems to enter accurate data for billing cycles and tracking participant progress.
The position is securely funded for 18 months while the HRSN Program Manager builds the program towards long term viability. The viability of the program is conditional on the success of the program by leveraging capacity building funds and developing a fee for service model with the new Medicaid waiver to bring revenue to sustain the department. The HRSN program is funded because of Oregon’s federal Medicaid 1115 waiver, to support individuals and families eligible for housing and other assistance through the Medicaid (Oregon Health Plan) program. Long-term success for this role will be dependent on program success within the first 18 months.
Primary Responsibilities
· Collaboratively work with the HRSN Program Manager, CCO’s, and agency partners to conduct outreach and engagement, intake, and assessment, focusing on those who are most vulnerable.
· Develop individualized case management plan.
· Provide advocacy for homeless individuals or families when they encounter housing barriers.
· Assist in identifying appropriate housing search and completing housing applications.
· Develop move-in and rent plans.
· Housing tenancy support.
· Assist referred individuals with accessing resources.
· Maintain complete client records, daily activity logs, mileage logs, and other reports as directed.
· Comply with HIPAA and state regulations regarding privacy and security.
· Establish and maintain positive, productive working relationships with mental health programs, shelter programs, police (and other local officials), and providers of services and resources to homeless neighbors.
· Attend team meetings, case conferences, training workshops and community meetings as needed.
· Client data entry information using Unite Us platform.
· Other duties as assigned by the HRSN Program Manager and the SHS Director.
Qualifications
· Bi-lingual in English and Spanish language required.
· High school diploma or equivalent, with a minimum of 4 years of related experience. Bachelor’s degree in public health, healthcare services, social work, or related field preferred.

· Has basic computer skills, use of Microsoft Office suite of programs, email, and related technology; must maintain related data tracking systems, including case notes.

· Possesses a valid driver’s license, proof of insurance.
· Able to work flexible hours, sometimes working on weekends.
