Your Mission
As a PT Social Worker, you'll partner closely with the SW/CM to drive strong resident outcomes by supporting each person's physical, emotional, and psychosocial well-being. This role exists to keep transitions smooth, communication clear, and resident rights front and center-every single day.
Key responsibilities include assisting with discharge planning, coordinating communication between residents, families/representatives, and the interdisciplinary team, and advocating for resident rights in line with Fellowship standards.
Required qualifications/skills:
- Bachelor's degree in Social Work
- Minimum two years' experience in geriatric care
- Excellent interpersonal/public relations skills; strong organization
- Ability to counsel individuals in crisis; knowledge of senior community resources
- Intermediate computer skills; working knowledge of the MDS
- Understanding of Medicare/MA/managed care/private pay basics; medical terminology

By day's end, you've helped keep plans on track, questions answered, and the team moving forward with confidence-then you wrap up knowing your 25 hours each week truly count.

Salary : $34 - $38
