Summary/Objective
The Patient Navigator is responsible for coordinating the provision of patient care services for newly diagnosed cancer patients throughout the healthcare system that are referred to the Baptist Cancer Network.
Essential Job Duties

Communicate with patients care teams, caregivers and family as needed. Refers patients to appropriate resources as needed. Documents all communication appropriately in the EMR.
Collaborates and facilitates communication among members of the multidisciplinary cancer care team to prevent fragmented or delayed care that could adversely affect patient outcomes.
Assist patients with understanding their insurance coverage, navigating billing issues, and identifying potential financial assistance programs.
Assesses patient needs upon initial encounter and periodically throughout navigation, matching unmet needs with appropriate services and referrals and support services, such as dietitians, providers, social work, genetic counseling, chaplain, clinical trials and financial services
Assist with care planning that includes referrals to home health, rehabilitation services, drug assistance programs and care including resources for advanced directives, considering and identifying palliative care and hospice resource options and identifying legal consultation resources. Works closely with the social worker and chaplain in coordinating services.
Participates in the tracking of metrics and patient outcomes, in collaboration with the Director, Clinical Manager and Oncology Informatics Nurse, to document and evaluate outcomes of the navigation program.
Required Education and Experience
Associates degree in a related healthcare field; i.e. Radiation Therapy, Radiologic Technology, Social Work
Required License/Certifications
Commission on Cancer Recognized Non-Nursing Navigation Certification within 1 year of hire
