What are the responsibilities and job description for the CASE MANAGER position at Bhrags?
- Conduct intake and make assessments of residents’ needs.
- Develop individualized service plans with short and long term goals alongside residents.
- Monitor compliance with goals through regular weekly contacts.
- Maintain and organize an efficient filing system.
- Work closely with shelter staff to enforce rules and regulations for residents.
- Serve as an advocate with agencies and service providers to minimize barriers to residents receiving needed services.
- Follow-up with referral sources to ensure that clients receive and access necessary services in a timely manner.
- Coach clients in job prep, housing services, social services and other areas as needed.
- Represent BHRAGS ALLIANCE at city and statewide coalitions, task forces and committees as necessary.
- Participate in monthly staff meetings, case conferences and training seminars as indicated by Director of Social Services.
- Ensure that all clients assigned is personally interviewed, assessed, and begins case management.
- Perform other duties as assigned by the Case Manager Supervisor.
Bachelor’s degree or some college in Human Services or related field is preferred.
Counseling experience preferred.
Demonstrated sensitivity and experience with and/or knowledge of immigration and disenfranchised communities including individuals who identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, and individuals with mental health issues.
Must have comprehensive written and oral communication skills.
Ability to work under pressure, able to respond to a crisis situation, health or safety issue and take appropriate and immediate action.
Ability to be flexible with schedule as needed.
Sound knowledge of computer system and/or Microsoft Office applications.
Create ongoing activities to engage clients.
Help participants develop skills such as communication and problem-solving.
Plan, develop, and lead recreational activities for groups or individuals.
Bilingual English/Spanish is preferred.
Other certifications may be requested.
