What You'll Do
- Provide individual, family, and group therapy using evidence-based, recovery-oriented practices.
- Complete behavioral health assessments and develop individualized treatment plans.
- Deliver community-based case management and connect clients to community resources.
- Respond to crises and advocate for client needs.
- Collaborate with psychiatrists, nurses, peer support specialists, and community partners.
- Maintain accurate clinical documentation and ensure compliance with agency and regulatory standards.
- Participate in the FACT team's rotating on-call schedule.

Qualifications
Required
- Bachelor's degree in Social Work, Counseling, or a related field
- Ohio LSW or LPC license
- Valid Ohio driver's license
Preferred
- LISW or LPCC
- Experience working with individuals with severe and persistent mental illness
- Community-based behavioral health experience
