As a Care Coordinator, you will:
- Build trusting relationships with adults experiencing mental health and substance use challenges.
- Assess client needs and develop individualized plans to connect them with the right services and resources.
- Coordinate care with treatment providers, community partners, and support systems.
- Monitor client progress and advocate for timely, effective services.
- Help individuals overcome barriers, improve stability, and achieve their personal recovery goals.

Our Location
Our administrative office is located at 65 Messimer Drive, Newark, Ohio, with Care Coordinator opportunities available throughout Licking and Knox Counties. We're conveniently located just a short commute from Columbus, Lancaster, Zanesville, and Mount Vernon.
Required Qualifications:
- One to three years of experience providing care coordination or related services to individuals with mental health and/or substance use disorders
- Qualified Mental Health Specialist (QMHS)
- Valid Ohio driver's license
- BLS/CPR certification
- Strong written and verbal communication skills
- Basic computer, phone, and typing skills
Preferred Qualifications:
- Licensed Social Worker (LSW)
- Licensed Professional Counselor (LPC)
