The Outreach Case Manager plays a critical role in engaging and supporting individuals and families experiencing unsheltered homelessness. Through street outreach, relationship-building, and comprehensive case management, this position helps clients overcome barriers to housing, income stability, healthcare, and other essential services.

Working in a client-centered, trauma-informed, and non-judgmental manner, the Outreach Case Manager assists clients in developing individualized service plans, accessing benefits and community resources, and navigating the path toward permanent housing and self-sufficiency. This position requires flexibility, strong communication skills, and a commitment to serving vulnerable populations in both office and community settings.

This is a full-time position, 37.5 hours per week, working Tuesday and Wednesday 8:30 am - 4:30 pm, Thursday 12 pm- 8 pm, Friday and Saturday 8:30 am - 4:30 pm, with flexibility required to meet participant needs, including occasional evenings, weekends, and holidays.

What You'll Do:

  • Conduct street outreach and respond to community requests for intervention and engagement.
  • Build trusting relationships with individuals and families experiencing homelessness.
  • Provide individualized case management and ongoing support to help clients achieve housing, health, and income stability.
  • Assist clients in obtaining vital documents, including identification cards, birth certificates, Social Security benefits, disability benefits, and other resources.
  • Support clients in applying for housing programs, including Rapid Re-Housing (RRH), Permanent Supportive Housing (PSH), Housing Choice Vouchers (HCV), and other community resources.
  • Connect clients to vocational services, employment resources, job coaching, healthcare, mental health treatment, and substance use services.
  • Provide information and education on harm reduction, health and safety practices, and available community support services.
  • Coordinate warm handoffs and referrals to long-term case management and service providers.
  • Distribute essential engagement and relief items such as hygiene products, transportation assistance, clothing, and weather-related supplies.

Documentation & Reporting

  • Maintain accurate and timely client records, case notes, and HMIS documentation.
  • Track client progress and document outcomes related to housing, health, and income goals.
  • Prepare reports highlighting client outcomes, successes, challenges, and program performance.
  • Ensure compliance with agency policies, procedures, and reporting requirements.

Community Collaboration

  • Work collaboratively with internal teams, community partners, government agencies, faith-based organizations, businesses, and coalitions.
  • Participate in team meetings, case consultations, clinical supervision, and ongoing training opportunities.
  • Maintain effective communication with colleagues working various schedules and partner organizations.

Quality & Safety

  • Follow outreach safety protocols and best practices while working in community settings.
  • Adhere to organizational standards of care and quality management requirements.
  • Complete required training, including HMIS and other agency-designated professional development programs.
  • Support organizational goals through regular attendance, professionalism, and flexibility in assigned duties.

Qualifications

  • High School Diploma or equivalent.
  • Valid driver's license with an acceptable driving record.
  • Experience working with individuals experiencing homelessness, mental health challenges, and/or substance use disorders.
  • Proficiency with Microsoft Office 365.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
  • Strong organizational, communication, conflict resolution, and problem-solving skills.
  • Demonstrated ability to work effectively with diverse populations.
