BHcare is seeking a Supportive Housing Case Manager to join our team. This role is focused on helping families build safe, stable, and nurturing home environments while working toward reunification and long-term independence.
What You’ll Do:
• Complete intakes and assessments to evaluate eligibility for services.
• Develop, monitor, and update individualized care plans.
• Provide advocacy, crisis intervention, and referrals for counseling, substance abuse recovery, parenting, childcare, education, employment, and other support services.
• Coordinate with DCF social workers, treatment providers, and partner agencies to ensure family safety and stability.
• Conduct home visits (including unannounced visits) and provide transportation when needed.
• Maintain accurate case records, data entry, and prepare monthly reports.
• Participate in team meetings, supervision, and professional development.
• Support program development and community outreach efforts.

MINIMUM QUALIFICATIONS:
- Bachelor’s degree required.
- Knowledge of child welfare, substance abuse, welfare, or supportive housing system.
- Knowledge of culture of addiction.
- Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
- Bilingual ability a plus.
- Must be able to obtain and maintain the certified domestic violence counselor certification per CGS 52-146k before providing direct service to clients, followed with six-hour recertification yearly
- Must have and maintain a valid driver’s license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization’s auto insurance plan.
LICENSURE /CERTIFICATIONS:
- Must be able to obtain and maintain BLS (Basic Life Support) certification
