About Beyond Home Health Care
Beyond Home Health Care is an approved provider with the New Jersey Division of Developmental Disabilities (DDD), delivering in-home therapeutic services to individuals with intellectual and developmental disabilities (I/DD) across New Jersey. We support participants enrolled in the Supports Program and Community Care Program (CCP) by providing individualized care that promotes independence, community participation, and quality of life.
Position Overview
We are seeking a licensed Occupational Therapist to provide individualized, in-home and community-based OT services to adults with developmental disabilities. The OT will evaluate functional performance, develop person-centered treatment plans, and deliver interventions that maximize independence in activities of daily living (ADLs), instrumental activities of daily living (IADLs), sensory processing, fine motor skills, and community participation, in accordance with NJ DDD guidelines.
Key Responsibilities
Conduct comprehensive evaluations and develop individualized treatment plans aligned with each participant's Individualized Service Plan (ISP) and person-centered goals.
Deliver one-on-one therapy services in the participant's home and community settings.
Document services in compliance with DDD documentation standards, including session notes, progress notes, and outcome measures, submitted within required timeframes.
Collaborate with Support Coordinators, families, caregivers, and the interdisciplinary team to ensure continuity of care.
Provide caregiver training and home programs to reinforce therapeutic goals between sessions.
Adhere to all DDD policies, including the DDD Provider Policies & Procedures Manual, reportable incident requirements (Danielle's Law, Stephen Komninos' Law), and HIPAA confidentiality standards.
Maintain accurate billing records consistent with DDD Fee-for-Service rates and authorizations.
Complete required DDD-mandated trainings (Pre-Service, Preventing Abuse/Neglect/Exploitation, Danielle's Law, Stephen Komninos' Law, etc.).
Required Qualifications
Current New Jersey license as an Occupational Therapist (OT) issued by the NJ State Board of Occupational Therapy.
Graduate of an ACOTE-accredited occupational therapy program.
NBCOT certification preferred; required for new graduates.
Minimum of one (1) year of clinical experience preferred; new graduates with relevant fieldwork considered.
Current CPR/BLS certification.
Clean criminal background check and fingerprinting per NJ DDD requirements.
Reliable transportation, valid NJ driver's license, and current auto insurance.
Ability to pass DDD-required background checks including the Central Registry of Offenders Against Individuals with Developmental Disabilities (CRoff).
Preferred Qualifications
Experience working with individuals with intellectual and developmental disabilities, autism spectrum disorder, or complex medical needs.
Familiarity with NJ DDD Supports Program and Community Care Program service definitions.
Bilingual (Spanish, Haitian Creole, or other languages a plus).
