Job Summary


GENERAL SUMMARY:   The QIDP is responsible for developing, implementing and monitoring each assigned client's active treatment program. The QIDP is responsible for the overall management of the assigned work location.

 

ESSENTIAL DUTIES AND RESPONSIBILITES includes the following.  Other duties may be assigned.

 

 

  1. Individual Habilitation Plan, Medical Record and Active Treatment

 

Prepare conduct, complete and implement Annual, Special and 30-day Individual Service Plans according to written Policies and Procedures for assigned caseload.

 

Conduct client ISP’s staffing’s according to written Policies and Procedures for assigned caseload.

 

Complete client ISP's after staffing’s according to written Policies and Procedures for assigned caseload.

 

Implement client programs and train all appropriate staff according to written Policies and Procedures for assigned caseload.

 

Attend, review and sign-off on ISP's conducted.

 

Ensure daily documentation is completed accurately. Implement corrective measures if not complete [re-training or discipline].

 

Complete Monthly Progress Notes on each client objective, reviewing and making changes as outlined in Policy and Procedures Manual for assigned caseload. Retrain staff as needed.

 

Review Resident Interdisciplinary Notes monthly and include documentation of monthly visit to residents Developmental Training Program for assigned caseload.

 

Participate in Quality Assurance Surveys – DT/work program and home/residential.

 

Follow up on medical, dietary, work, activity concerns, etc. as needed

 

Ensure active treatment is conducted with residents through supervision, training, follow-up and example. Ensure residents participate in off-ground activities.

 

QIDPs will be assigned to spend 20% or more of their work-time in a direct care or support role, must complete the Beverly Farm Foundation DSP training-program, and have their name added to the Health Care Worker Registry as a DD Aide.

 

Ensure completion and documentation of residents' Activities of Daily Living (ADLs)

 

Facilitate meaningful community outings in alignment with individualized support plans and regulatory standards.

 

Complete regular quality assurance file audits and purge client files of old information for assigned caseload.

 

Ensure all evaluations, inventories, assessments and routine information is current in residents files/ record for assigned caseload.

 

Ensure adequate, seasonal well-fitting clothing is available for each assigned client and that clients are well-groomed.

 

 

  1. Supervision:

 

Supervise and ensure completion of work by DSP and shift supervisors on assigned shifts and maintain good working relationship.

 

Ensure schedules are maintained that provide adequate staff coverage to meet client needs requiring as little overtime as possible - as assigned.

 

Approve ordering of supplies which are needed to provide satisfactory living environment for clients as assigned.

 

Schedule, train, supervise and evaluate CPC's, Shift Supervisors and new employees as needed. Train on and oversee DHS Training competencies as needed for new employees

 

Do building walk-throughs with outgoing and oncoming supervisors as appropriate and review Communication Log.

 

Ensure shift meetings and inservices are conducted in the building on a regular basis, that inservice sheets are completed and forwarded to appropriate personnel as assigned.

 

Ensure that staff work hours are correct and turned in to the Business Office by 10:00 am each Tuesday for payroll purposes.

 

Ensure that Census Report is complete in quantum and Leave/Arrive notices are completed correctly and turned in to the Business Office monthly by 10:00AM on the 1st of each month.

 

Complete staff counseling and necessary disciplinary actions according to guidelines in Beverly Farm Foundation Employee Code of Conduct.

 

 

  1. Building Management:

 

Ensure building cleanliness, maintenance and safety by completing daily walk-throughs and documenting any problems with communication to appropriate personnel as assigned.

 

Maintain and review regularly as assigned a list of needed repairs or corrections for the Maintenance Department. Report any longstanding uncorrected problems to the Unit Director.

 

Ensure grounds around building are free from safety hazards to clients and litter-free. Document and report problems to appropriate personnel.

 

Ensure furniture in building is free of safety hazards to clients and in proper working condition. Document and report any problems to appropriate personnel.

 

Ensure that family/guardian concerns/suggestions have appropriate follow-up completed.

 

  1. Miscellaneous

 

Work the floor as necessary and cover for staffing needs.

 

Maintain budget as developed for the work location.

 

Evaluate future needs/wants of the current work location.

 

Maintain professional competency by completing a minimum of 12 hours of continuing education annually as required by Illinois state guidelines.

 

Qualifications


Qualifications

Education/Experience:

Bachelor's Degree in a Human Service Field with at least one year of experience working directly with persons with an intellectual disability or other developmental disability.

 

Skills Requirements:

 

  1.  Highly proficient computer skills and comfortable with software programs such as Microsoft Office Suite (word, excel, access, power point) or similar software. Advance knowledge and use of Outlook.
  2. Interpersonal Skills – Focuses on solving conflict, maintaining confidentiality; Ability to effectively present information and respond to questions from the Administrators, Unit Directors, Clinical, Human Recourses, Employees, Clients and the General Public.
  3. Oral Communication – Speaks clearly and persuasively in positive or negative situations; Listens and gets clarification; responds well to questions; Actively participates in organizational meetings and events. 
  4. Written Communication Skills – Writes clearly and informatively; Edits work for spelling and grammar; Varies writing style to meet needs.
  5. Teamwork Skills – Balances team and individual responsibilities; Exhibits objectivity and openness to others’ views; Gives and welcomes feedback; Contributes to building a positive team spirit; Able to build morale and group commitments to goals and objectives.
  6. Professionalism – Approaches others in a tactful manner; Reacts well under pressure; Treats others with dignity and consideration regardless of their status or position; Accepts responsibility for own actions.

 

Abilities Requirements:

 

Mathematical Skills:

Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals. 

 

Reasoning Ability:

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or report form. Ability to deal with problems involving several concrete variables in non-standardized situations. Able to use tangible and intangible information to come to a result.

 

Technology Skills:

Utilizes computer to accomplish job tasks. Computer literacy essential in word processing, spreadsheet applications, and databases. Knowledge of Outlook or similar program.

While performing the duties of this job, the employee is regularly required to sit, stand, walk, bend, squat, crawl, and reach. Employee will occasionally use a mechanical lift, climb stairs and ramps. The employee will frequently and occasionally lift and/or move up to or over 75 pounds. From time to time, employee may be required to work long hours. The employee is frequently required to use both hands, push/pull, use sharp objects, and talk. The noise level in the work environment is usually moderate. On occasion, noise level may be very loud. On occasion the work environment may be cold and hot.

 

Specific vision abilities required by this job include close vision, distance vision, and peripheral vision. Specific hearing abilities required by this job include the ability to hear when communicating with employees and using a phone line.
