Essential Duties and Responsibilities
- Conduct nursing assessments and develop individualized care plans.
- Provide skilled nursing services in accordance with physician orders and agency policies.
- Administer medications, treatments, and other nursing interventions as prescribed.
- Monitor client health status and report changes to physicians, supervisors, and family members as appropriate.
- Complete accurate and timely documentation of all services provided.
- Educate clients, families, and caregivers on health conditions, medications, and care plans.
- Participate in care coordination with interdisciplinary team members.
- Ensure compliance with state regulations, agency policies, and client rights.
- Assist with supervisory visits and evaluations of direct care staff when required.
- Maintain client confidentiality and adhere to HIPAA regulations.
- Attend required trainings, meetings, and continuing education activities.
Pay: $35.00 - $38.00 per hour
