Responsibilities:
- Provide client-centered care in home and community settings (e.g., ambulating, grooming, dressing, toileting, eating)
- Support clients’ independence and personal goals using creativity and empathy
- Collaborate with team members, clients, and families for effective care coordination
- Identify and solve challenges proactively to ensure safety and quality care
- Transport clients safely to community activities or appointments when applicable
Qualifications:
- High school diploma or equivalent; 18 years old; eligible to work in the U.S.
- Compassion, patience, positive attitude, and strong communication skills
- Ability to work independently and safely assist clients (lift up to 50 lbs, walk long periods, assist with mobility, respond to elopement)
- Reliable transportation for roles involving client transportation
