This is a full-time, on-site Care Coordinator role based in Fargo, ND. The Care Coordinator will support clients in accessing mental health and addiction recovery resources and services. Responsibilities include assessing client needs, creating personalized care plans, and coordinating with healthcare providers and community resources to ensure effective care delivery. The Care Coordinator is also responsible for maintaining accurate records, advocating for client needs, and fostering positive relationships to facilitate recovery and well-being.
- Strong organizational and time-management skills for coordinating client care plans effectively.
- Excellent communication and interpersonal skills to engage with clients, healthcare providers, and community partners.
- Knowledge of mental health and addiction recovery resources and understanding of evidence-based practices.
- Ability to work onsite, offering hands-on support to individuals and communities in need.
- Ability to adapt to the needs of a diverse demographic and uphold values of inclusivity and empathy.
- Proficiency in administrative tasks, maintaining detailed records, and reporting outcomes.
- Bachelor's degree in Social Work, Psychology, or a related field preferred; relevant experience is also welcomed.
