What are the responsibilities and job description for the PRN Care Partner position at Better Living Management Services, Inc?
JOB SUMMARY
This position is a member of the health care team who has received caregiving training and has been deemed competent
in these skills. This position works under the supervision of the Health & Wellness Director partnered with the Executive
Director/Assistant Executive Director. Care Specialists perform a variety of services and tasks necessary to maintain and
support residents in their activities of daily living including personal cares, delegated nursing functions, social
interactions/activities, and homemaking.
ESSENTIAL FUNCTIONS
1. Provide servicesthat are described in written plans of care and service schedulesfor which the Care Specialist
has been oriented.
a. Provide personal care services as described in the resident’s plan of care including bathing, dressing,
grooming, meal set-up, toileting, positioning, transferring, mobility, and any other personal care.
b. Provide delegated nursing services as described in each resident’s plan of care including assistance with
medications, skin care, range of motion, maintenance therapy, respiratory assistance, assistance with
prosthetic and orthotics, necessary observation and monitoring, wound care, and other treatments.
c. Provide homemaking tasks as described in the daily schedule/service plan, including cleaning and
straightening resident’s apartments, bedrooms, bathrooms, kitchens, living rooms, cleaning and
straightening the congregate living and dining areas, washing, folding, and putting away resident’s laundry
and linen.
d. Serve meals and snacks as assigned. Provide set up and eating assistance as needed. Assist with clean up
after meal service and clean work areas including equipment, dishes, and utensils.
e. Provide psychosocial support for residents by daily kind interpersonal interaction, assistance with
scheduled and informal recreational activities, and a sincere interest in the emotional well-being of each
resident.
f. Respond to and report significant changes in resident’s health status including but not limited to: choking,
chest pain, fever, change in level of consciousness or behavior, hyper/hypoglycemia, abnormal vital signs,
abnormal eating or bowel patterns, weight loss.
g. Review the service plans, service schedules, medication services, care plans and program policies; tell the
Executive Director/Health & Wellness Director if you have questions or have not been trained adequately to
provide all the services described in the plans and policies.

2. Document all services given and other information based on instructions from the Health & Wellness Director, or
designee, and policies/processes for community.
a. Document all services/tasks, completed or declined, accurately according to policy.
b. Document any new information, changes, useful information, and informal communication in the
communication book. Read communication book daily.
3. Participate as a team member at the assisted living community to ensure that residents get their personal care,
spiritual, psychosocial, and environmental needs met within a safe environment.
a. Attend all mandatory meetings and in-services unless excused by leadership team member.
b. Complete all education and updated/new policies assigned by the leadership team.
c. Communicate information to the Health & Wellness Director about the work you do that may help to revise
the plan of care for a resident and improve the quality of the resident’s services; communicate information
about the operation of the site that will help the team become better employees and make the facility a better
place to live.
d. Be a team player and assist other staff as needed.
e. Demonstrate good customer service skills including treating all residents, family members, visitors, and other
staff with respect and courtesy.
4. Follow all rules, policies and procedures that apply to the services provided at the community.
a. Understand, demonstrate, and know where to find needed information about policies.
DESIRED SKILLS AND ABILITIES
1. 1-3 years of experience in senior care preferred.
2. Able to work independently as well as part of a team.
3. Understand, respect, and maintain confidentiality about the resident’s with whom they work.
4. Ability to properly use chemicals understands and adheres to facility Right to Know Policy and possesses
knowledge of SDS for chemicals used to perform functions of the job.
5. Ability to observe Universal Precautions and Infection Control procedures.
6. Ability to recognize and act on safety hazards – wipe up spills or wet spots off floor, keep hallways uncluttered
when using cleaning equipment.
PHYSICAL DEMANDS & WORKING CONDITIONS
• Works primarily indoors in a climate-controlled setting.
• Possible exposure to unpleasant odors.
• Possible exposure to chemicals as indicated in the MSDS manual.
• Continuous exposure to clients and/or family members who may be under stress.
