We are seeking a bilingual English/Spanish Home Care Coordinator to join our dynamic team. The ideal candidate is organized, dependable, detail-oriented, and able to make sound decisions in a fast-paced home care environment.

The Home Care Coordinator works closely with patients, caregivers, and office staff to ensure that daily staffing and scheduling needs are effectively coordinated and that patients receive consistent, reliable care.

Key Qualifications

• High school diploma or equivalent required; business or healthcare experience preferred.
• Minimum of 1 year of home care experience required.
• Minimum of 1 year of customer service experience preferred.
• Bilingual English/Spanish preferred.
• Valid driver's license required.
• Reliable, insured vehicle required.
• Must maintain valid automobile insurance and a reliable vehicle suitable for business-related travel.
• Ability to travel between BHHC's Rockville Centre and Forest Hills locations as needed.
• Strong computer skills and ability to learn and utilize agency software.
• Experience with HHA Exchange preferred.
• Strong decision-making and problem-solving skills.
• Ability to make sound staffing and scheduling decisions based on patient needs.
• Excellent communication and interpersonal skills.
• Strong organizational and time-management skills.
• Detail-oriented with strong documentation skills.
• Ability to work independently while functioning effectively as part of a team.
• Ability to prioritize multiple responsibilities in a fast-paced environment.
• Must be able to read, write, and communicate effectively in English.
• Home care or healthcare experience strongly preferred.

Key Responsibilities

• Coordinate staffing and assign field staff to meet patient care needs.
• Maintain accurate and up-to-date schedules for patients and field staff.
• Arrange appropriate coverage for caregiver call-outs, absences, and schedule changes.
• Communicate effectively with patients, caregivers, and office personnel regarding scheduling and staffing needs.
• Monitor staffing changes and respond promptly to coverage needs.
• Maintain accurate patient and staffing documentation.
• Conduct bi-weekly patient progress calls and maintain positive relationships with patients and families.
• Identify and communicate staffing or patient service concerns to appropriate management.
• Support the smooth day-to-day operations of the agency.
• Participate in the agency's on-call rotation as assigned.
• Work a rotating schedule as required to meet operational and patient-care needs.
• Report to and work from BHHC's Rockville Centre and Forest Hills locations, depending on operational needs.
• Travel between locations when assigned or required.
• Promote positive relationships and professional communication among patients, caregivers, and agency staff.

Schedule & Work Locations

This is an in-person position requiring flexibility to work at both BHHC office locations:

Rockville Centre, NY
Forest Hills, NY

Work hours will rotate between:

• 8:00 AM–4:00 PM
• 9:00 AM–5:00 PM

The assigned schedule and location may vary based on agency operational needs.

A valid driver's license and reliable, insured vehicle are required due to the need for travel between BHHC locations and other business-related activities.

Company-paid parking is provided.
