Better Care at Home is seeking a motivated and relationship-driven Community Liaison to expand our community presence and referral partnerships throughout the Indiana area.
In this role, you will focus on building and maintaining relationships with hospitals, assisted living communities, rehabilitation centers, case managers, and community organizations. The Community Liaison will serve as the primary connection between Better Care at Home and the local community, helping increase awareness of our services and supporting families who need home care assistance.
This position combines community outreach, relationship development, and service education to help grow referrals and strengthen our presence within the healthcare community. You will actively represent Better Care at Home at community events, networking meetings, and outreach visits while helping families understand the home care services available to them.
This position plays a vital role in connecting individuals and families to quality home care services in our community. Preference will be given to candidates with experience in healthcare, home care, senior services, discharge planning, case management, or medical referral environments, as understanding healthcare systems and referral processes is essential for success in this role.
Key Responsibilities
Community Outreach & Relationship Building
- Conduct in-person outreach visits to hospitals, rehabilitation facilities, assisted living communities, senior centers, and community organizations.
- Develop and maintain strong relationships with:
- Hospital discharge planners
- Case managers
- Social workers
- Senior living communities
- Veterans organizations
- Community service providers
- Represent Better Care at Home at community events, health fairs, and networking opportunities
Referral Development & Partnership Growth
- Develop and maintain referral partnerships to support business growth
- Follow up regularly with referral partners to maintain strong professional relationships
- Track referral sources and maintain accurate outreach records
- Support initiatives to expand services within Marion County and surrounding areas
- Identify new referral opportunities within the community
Community Education & Resource Support
- Educate families, healthcare professionals, and community partners about Better Care at Home services
- Provide information about available home care options, including private pay services and community resources
- Assist families in understanding how home care services can support independent living and improved quality of life
- Serve as a knowledgeable resource for community members seeking home care assistance
Marketing & Community Engagement
- Distribute marketing materials and brochures to referral partners and community organizations
- Assist with planning and promoting community events such as health fairs, senior outreach programs, and informational sessions
- Support brand awareness initiatives through community engagement and outreach activities
- Collaborate with the marketing team to support outreach campaigns and initiatives
Qualifications
- Previous experience in community outreach, healthcare marketing, business development, or relationship management preferred
- Experience working in healthcare, home care, senior services, hospital systems, or medical referral environments strongly preferred
- Strong interpersonal and networking skills
- Excellent communication and presentation abilities
- Ability to build and maintain professional relationships within the community
- Reliable transportation and willingness to travel locally for outreach visits
- Organized, self-motivated, and goal-oriented
- Ability to work independently while supporting team growth initiatives
