The Intensive Case Manager (ICM) to provide hands-on outreach and crisis support to individuals in emergency shelters, including hotels and motels. In this fast-paced role, you’ll work closely with local partners to quickly move individuals from temporary placements into stable housing while addressing critical health and social needs.

The Intensive Case Manager (ICM) to provide hands-on outreach and crisis support to individuals in emergency shelters, including hotels and motels. In this fast-paced role, you’ll work closely with local partners to quickly move individuals from temporary placements into stable housing while addressing critical health and social needs.
Onsite, full-time (40 hr/week), Non-exempt position
Pay Rate: $23/hour based on individual skills and experience
Schedule: M-F, 8am-4:30pm
Crisis Response & Outreach
- Provide on-site support in emergency shelters and hotel placements
- Conduct intakes, assessments, and immediate service planning
- Deliver intensive, hands-on case management and advocacy
Housing Navigation
- Partner with Schenectady County DSS to expedite housing placements
- Coordinate appointments and attend with clients as needed
- Support rapid transitions from shelter to permanent housing
Collaboration & Care Coordination
- Work closely with Social Work and Medical teams to address whole-person needs
- Build strong relationships with community providers and housing resources
Data, Compliance & Reporting
- Maintain accurate documentation and HMIS data (Coordinated Entry, ES, CM)
- Complete required reports and ensure compliance with all regulations
- Prepare for audits and maintain organized client files
- Experience in homelessness, crisis response, or intensive case management
- Strong communication, organization, and problem-solving skills
- Ability to thrive in a fast-paced, field-based environment
- Knowledge of community resources and housing systems
- Experience with HMIS or similar data systems preferred
