RN CLINICAL MANAGER JOB DESCRIPTION

 

Job Summary: The RN Clinical Manager provides clinical leadership, oversight, and direct patient care for Home Health and Hospice services. This position is responsible for coordinating and managing patient care, supervising clinical staff, and ensuring compliance with agency policies, quality standards, and state and federal regulations. The RN Clinical Manager performs skilled nursing services in home, community, and on-campus settings while supporting patients and families through all phases of care. This role promotes high-quality, compassionate, and patient-centered care through effective case management, interdisciplinary collaboration, and clinical oversight.

Job Relationships:

  1. Reports to: Hospice Director
  2. Supervisory Responsibilities: RN Case Managers, LPNs, Hospice Care Aides and Homemakers. 
  3. Internal Relationships: Other RN Case Managers, Scheduling Coordinator, Bethesda Social Service departments, Volunteer Coordinator, Housing Managers and Coordinators, Contracted Therapists and Medical Director.
  4. External Contacts: Medical Director, Physicians, Pharmacists, Health Service personnel in surrounding area, Kandiyohi County Family Services Social Workers and Public Health Nurses, Nursing Home Social Workers, Hospital Discharge Planners, Insurance Company personnel, Patients and Family members, and other community providers.

Hours of Work: Monday through Friday; 8:00 a.m. to 4:30 p.m. Must be flexible for patient needs. Rotating on-call duties, with evening and weekend visit expectations.

Dress Code: Street clothing or nursing uniform and identification badge.

Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity.)

  1. Education: 
    1. Graduate of an accredited school of nursing.
    2. Current Minnesota registration as a Registered Nurse. Must maintain licensure.
    3. Current CPR Certification or ability to immediately obtain certification
  2. Experience: 
    1. Strong proficiency in nursing skills.
    2. Home Health and Hospice experience preferred.
    3. Previous management experience preferred.
    4. Basic Computer skills.
  3. Personal:

    12. Own vehicle, current valid driver’s license, and proof of current automobile insurance.
    13. In accordance with CMS Vaccine Mandate, must provide proof of Covid-19 Vaccine or submit documentation for exemption
  4. Job Knowledge:

    5. Ability to assess patient’s condition with knowledge to provide necessary action.

    8. Computer knowledge.

Physical Demands of the Job: 

a. Repeated bending, squatting, stooping, sitting, climbing stars.

b. Occasional pushing/pulling movements of wheelchairs or patients weighing 100-250 pounds.

c. Frequent lifting over 25 pounds.

d. Frequent exposure to infection.

Principle Job Responsibilities:

Responsibility: Provide and monitor quality care.

Tasks:

  1. Follow State and Federal regulations and agency policies and procedures.
  2. Overview and scheduling of on-call. 
  3. Provide coordination of patient care to ensure continuity and quality care.
  4. Deliver skilled services efficiently and effectively to meet patient's and family's/caregiver’s needs.
  5. Coordinate and review Home Health and Hospice referrals with IDT team.
  6. Complete an initial, comprehensive, HOPE, and ongoing assessment of patient and family/caregiver to determine Home Health and Hospice needs. Provide a complete physical assessment and history of current and previous illnesses.
  7. Collaborate with physicians, other health care professionals, patients, and families/caregivers in developing a comprehensive coordinated plan of care, which includes objective goals for patient improvement in health and functional status. Evaluate plan of care on continuous basis, update physician and other therapies of changes in patient status and need for revision of plan of care. Include the patient and family/caregivers in the planning process.
  1. Teach the patient and family/caregiver self-care techniques as appropriate. 
  2. Provide medication, diet and other instructions as ordered by the physician and recognize and utilize opportunities for health counseling with patients and families/caregivers. 
  3. Ensure that physician orders are communicated to the agency’s staff with accuracy and thoroughness and understanding.
  4. Ensure that arrangements for equipment and other necessary items and services are available.
  5. Provide and maintain a safe environment for the patient, family/caregiver, and all staff or volunteers.
  6. Initiate case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  7. Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
  8. Collaborate with Director and Human resources to provide strong orientation and training of team members. 
  9. Assist with the scheduling of patients to all staff.
  10. Review documentation of all staff.
  11. Supervise all staff and complete supervisory visit documentation as required. Give feedback to all staff when needed.
  12. Follow infection control guidelines to prevent the spread of disease and infection.
  13. Provide assistance to all staff in new or difficult situations.
  14. Select appropriate patient for Telemonitoring. Use and monitor results from Telemonitoring system.
  15. Assist with development, implementation, review, and revising of policies and procedure that guide and support the provision of services. 
  16. Assist with development, implementation, review and revising of a Quality Assurance/Improvement Program.

Responsibility: Appropriately and safely perform/oversee procedures and treatments as ordered by Physician.

Tasks:

  1. Procedures and treatments include but not limited to:
    1. Medication set-up and administration.
    2. Dressing changes.
    3. Catheterizations and catheter care.
    4. Colostomy care.
    5. Tube feedings.
    6. IV administration and site care.
    7. Vital Signs.
    8. Respiratory therapy including nebulizer treatments.
    9. Blood glucose monitoring.
    10. Blood draws.

Responsibility: Perform all documentation with accuracy and timeliness.

Tasks:

  1. Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition, pain/symptom management, and care given. 
  2. Complete necessary paperwork for admissions, transfers, deaths, and discharges.
  3. Complete all required assessments.
  4. Document each visit in medical record.
  5. Complete Comprehensive and OASIS assessment when required.
  6. Complete Home Health and Hospice Care Plans prior to the initiation of those services.
  7. Complete Communication Log after communications with patients, family, health care providers, or staff.

Responsibility: Provide on-call and after-hours nursing services on rotating basis when needed.

Tasks:

  1. Carry on-call cell phone at all times when on-call.
  2. Respond to calls received, assess the situation, and provide direction.
  3. Document all phone calls.
  4. Perform Skilled Nursing Visits as needed.
  5. Participates in evening, weekend, and holiday rotation as needed/scheduled.

Responsibility: Marketing agency services

Tasks:

  1. Present self in a professional manner.
  2. Take referrals as needed.
  3. As opportunities arise, promote agency services to discharge planners, physicians, potential patients/families, and the general public.

Professional and Personal Growth:

  1. Report for and leave the work area on a timely basis.
  2. Participate in annual performance evaluation process.
  3. Write yearly personal goals.
  4. Participate in all required in-service programs at Bethesda.
  5. Maintain a professional conduct and pleasant, friendly attitude towards staff, residents, and visitors.
  6. Be willing to learn and change to meet needs that arise.

Salary : $74,720 - $84,146
