Description
JOB SUMMARY:
The Staff Nurse LPN will be responsible for assessing individual resident’s needs, as well as planning, implementing, and evaluating care for a defined group of residents, under the direct supervision of a Registered Nurse. LPNs coordinates many of the services that affect residents and their families in the protection and promotion of wellness.
- Provides professional nursing care to the residents in their assigned area and in other areas as needed.
- Administers medications and does treatments as ordered by a Physician.
- Reviews Physician Orders and ensures accuracy of Medication Sheets.
- Assists in the development of resident care plans.
- Goes on routine rounds with all patients in assigned unit notes any changes in condition.
- Documents all information regarding a resident’s physical, emotional, social, and spiritual wellbeing in the electronic charting system. Charting is done electronically in all Bethany St. Joseph Corporation Facilities.
- Prepares residents for admission, transfer, or discharge.
- Instructs residents and their families in self-help and self-care.
- Supervises C.N.A.’s working in assigned unit and directs them in their job duties.
- Ensures that resident rights are maintained at all times and reports any suspected violations immediately according to facility policy.
Job Responsibilities:

- Routine rounds with all patients in assigned unit.
Requirements:
- Must be currently licensed to practice in the state of Wisconsin as a Licensed Practical Nurse (LPN / LVN).
- Experience working in Assisted Living or Long-Term Care is beneficial, but not required. We will train the right fit for our team!
- Must be certified in C.P.R. or have the ability to obtain certification within 6 months.
