The Referral Coordinator is responsible for managing the referral and admission process for the organization’s long-term care and rehabilitation facilities. This role works closely with area hospitals to receive, review, and coordinate patient referrals, ensuring timely approvals and smooth transitions into our care facilities.
JOB FUNCTIONS:
- Serve as the primary point of contact for hospital and external provider referrals.
- Review incoming referrals for completeness and appropriateness.
- Coordinate approvals and admissions with the organization’s long-term care and rehabilitation facilities.
- Communicate with patients, families, and hospital staff to facilitate smooth transitions.
- Maintain accurate documentation of referrals and admissions in electronic health record systems.
- Utilize an electronic health record system to manage referral and admission workflows.
- Collaborate with clinical and administrative teams to ensure compliance with organizational policies and regulatory standards.
QUALIFICATIONS:
- Preferred: Registered Nurse (RN) or Licensed Social Worker (LMSW, LCSW) with relevant experience.
- Strong preference for candidates with long-term care and rehabilitation referral and admission experience.
- Excellent organizational, communication, and interpersonal skills.
- Proficiency with electronic health record systems; experience with Epic or PointClickCare is highly beneficial.
- Ability to work independently and as part of a team in a fast-paced healthcare environment.
WORKING CONDITIONS:
Primarily office-based with occasional travel to area hospitals or facilities. May involve some evening or weekend work as needed to facilitate admissions, but primarily Monday thru Friday during the hours of 8:00am and 5:00pm.
