What You’ll Do
- Complete social service assessments and develop individualized care plans
- Participate in care conferences and collaborate with the interdisciplinary team
- Coordinate admissions, discharges, and transfers
- Provide emotional support and guidance to residents and families
- Connect residents with community resources and services
- Maintain accurate, compliant documentation
- Advocate for resident rights, dignity, and quality of life
- Bachelor’s or Master’s degree in Social Work
- Active LSW in South Dakota (or ability to obtain quickly)
- Experience in long-term care, healthcare, or discharge planning preferred
- Strong communication and organizational skills
- A team-first mindset and passion for senior care
