Position: Registered Nurse
REPORTS TO: Clinical Supervisor/RN Case Manager
Position Summary
Performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and Agency.
Provide care to assigned clients by assessing needs, implementing nursing care plans, providing appropriate interventions, making appropriate revisions and evaluating outcomes of care.
QUALIFICATIONS
- Graduate of an accredited school of professional nursing.
- Current license to practice as a Registered Nurse in the state(s) of employment.
- Minimum of one (1) year experience in nursing
- CPR qualified.
- Demonstrated written, verbal and interpersonal communication skills.
- Competency in performing nursing assessments and procedures.
- Documents client care per policies and procedures.
- Recognizes the needs and concerns of people and maintains constructive working relationships when dealing with them.
- Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Has a good driving record.
- Must be able to safely perform the essential functions of the position without posing a direct threat to the health and/or safety of self or others in the work place.
ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
- Provides direct client care according to policies and procedures and productivity standards.
- Functions independently in the home setting according to established productivity standards.
- Delivers nursing care to clients, which included teaching, providing direct care, counseling, case management, referral advocacy, and monitoring client's ongoing heath issues.
- Seeks guidance and supervision from the Nursing Supervisor when issues or questions arrise.
- Documents findings accurately, in a timely manner, and on appropriate forms according to policies and procedures.
- Demonstrates the ability to care for a variety of clients and utilize supervision, consultation, case review and case conferencing when appropriate to achieve positive client outcomes.
- Provides accurate and timely timesheets per policy.
- Maintains and follows a written schedule, visit frequencies and physician orders for home visits to clients as assigned.
- Must maintain client confidentiality.
We are seeking a compassionate and experienced pediatric nurse to provide in-home tracheostomy care for an infant. The ideal candidate will have experience caring for pediatric patients with tracheostomies, demonstrate excellent clinical skills, and provide safe, attentive, and family-centered care. Responsibilities include tracheostomy care and suctioning, monitoring respiratory status, administering medications as prescribed, documenting care, and responding appropriately to emergencies. Current RN licensure and pediatric tracheostomy experience are preferred.
