SUMMARY: The Part-Time Outreach Assistant engages individuals and families experiencing unsheltered homelessness through consistent, relationship-based outreach in the community. The Part-Time Outreach Assistant meet individuals where they are, both physically and emotionally, building trust and rapport over time while connecting people to housing opportunities, healthcare, benefits, and supportive services.
This role prioritizes Housing First, harm reduction, trauma-informed care, and client choice, recognizing that stability and trust are foundational to successful housing outcomes.
The Part-Time Outreach Assistant collaborates with the Coordinated Entry system, housing providers, and community partners to assist individuals in transitioning from unsheltered homelessness into stable housing. This role requires flexibility, compassion, and strong engagement skills to support individuals facing complex barriers including trauma, behavioral health needs, and long histories of homelessness.

Able to work independently, as a team member, and work effectively with other community agencies in the best interest of the client, agency, and organization.ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
· Conduct proactive and responsive outreach to individuals experiencing unsheltered homelessness in encampments, parks, beaches, vehicles, abandoned structures, and other locations unfit for habitation.
· Respond to community requests for street outreach intervention
· Maintain accurate and timely documentation in HMIS and other required systems.
· Document outreach encounters, engagement progress, and housing outcomes through case notes and service entries.
· Complete homelessness verifications and required program documentation.
· Coordinate a warm handoff to long term case manager.
· Maintain strong working relationships with community providers including healthcare organizations, behavioral health providers, and housing partners.
· Provide supportive services in a non-judgmental manner.
· Distribute engagement items like bus passes, hygiene products, gloves, coats, hats, etc. to provide relief to people experiencing unsheltered homelessness.
· Document progress toward desired outcomes – housing, health and income stability. Maintain client related data tracking systems, including case notes and complete HMIS entries.
· Prepare case-related reports including outcomes, successes and challenges.
· Provide information, referrals, linkages, and advocacy to assist clients in accessing services and resources.
· Assist clients with procuring necessary documents and services such as identification cards, birth certificates, social security income, disability income.
· Identify appropriate options for clients such as supportive housing (RRH, PSH), HCV, etc. Assist with housing applications/eligibility paperwork.
· Attending weekly/monthly meetings with community partners and agency meetings is required.
· Working nontraditional hours is required and crucial that excellent communication be maintained with weekday staff and networking partners.
· Network with other agencies, coalitions, business, faith-based groups, and local community.
· Exercise high degree of safety cautiousness and application of community outreach safety guidance.
· Distribute engagement supplies including hygiene items, weather protection supplies, and transportation assistance when appropriate.
· Adhere to the standards of care to ensure that the identified quality measures are obtained.
· Supply data in a timely fashion to fulfill reporting requirements.
· Maintain documentation in accordance with policies and procedures.
· Attends training sessions, workshops, and seminars as requested by supervisor, or as needed for professional development. HMIS training required; must learn the applicable and required software reporting systems.
· May be required to use own vehicle on company business.

EDUCATION AND EXPERIENCE
· Bachelor's degree in human services; MA preferred
· Experience working with persons with MH and/or SUD
· Experience working with homeless or other disadvantaged population
· Proficient with Microsoft Office Suite, Outlook, and related software.
· Experience working with the homeless population
· Must be proficient with PCs and Microsoft Office Word & Outlook
· Understanding of Housing First philosophy
· Training in harm reduction, motivation interviewing, de-escalation techniques
