SUMMARY OF DUTIES
Responsible for supporting counseling staff in the Certified Community Behavioral Health Clinic. Will provide supporting duties including requesting and organizing clinical/medical records, completing supportive documents for counseling staff, providing support to counselors as needed.
RESPONSIBILITIES
- Assists in coordinating communication regarding care between clinical team, prescribers, and nursing staff
- Monitoring and contacting clients, to include those identified as high risk, with no recent contact or no next appointment scheduled
- Reaching out to collaterals and outside providers (PCP, insurance companies, inpatient, and detox facilities) involved in client care to request information
- Assists counselors in obtaining resources needed for group and individual sessions
- Obtains toxicology and lab reports for counselors
- Helps to complete court reports, completion of paperwork and other clinical data needed
- Coordinates and schedules necessary appointments
- Prepare/Stage documentation such as discharges, for counselors to complete
- Schedule and assigned duties are subject to change based on the needs of the program and the clients we serve.
- Performs all other duties as assigned.
QUALIFICATIONS
- High School Diploma or GED
- Some knowledge/experience with coordinating and collaborating between multiple person(s) or entities
- Ability and flexibility to learn and execute new tasks and processes
- Ability to ask for support and/or clarification when needed
- Great organization, problem-solving and time management skills
- Able to thrive in fast-paced environments
- BIPOC and Bilingual English/Spanish, Arabic, or other language strongly encouraged to apply.
