🩺 Role Overview
As a Primary Hospice RN Case Manager, you will:
- Manage and coordinate care for an assigned patient caseload
- Lead the interdisciplinary care team for each patient
- Provide symptom management and end-of-life care
- Educate and support patients and caregivers
- Help families define and achieve personal care goals
- Deliver care across home, nursing, and assisted living settings
✅ Requirements
- Active RN license
- Minimum 2 years of nursing experience
- Strong clinical judgment, assessment, and critical thinking skills
- Ability to manage multiple patients and priorities independently

- BLS / CPR – required
