Job Responsibilities:
- Conduct initial home visits to assess patient needs and develop the Aide Plan of Care
- Communicate with a patient’s physician and other team members to ensure delivery of appropriate patient care
- Supervise patient care provided by our Home Health and Personal Care Aides
- Participate in orientation, teaching, and evaluations of care providers
- Responsible for the review and maintenance of patient records, using telehealth modalities for early intervention of patient issues
- Must be a valid Registered Nurse licensed in New York State, preferably with two (2) years of experience
- Must have a valid driver’s license and transportation.
- Previous home care experience is a plus
- Bilingual Spanish and English language abilities is a plus
